FIHN originated in 2014 and has continued to grow its network and contract portfolio throughout the years. Frederick Integrated Healthcare Network, LLC- the Care Transformation Organization (CTO)
Frederick Integrated Healthcare Network, LLC is proudly participating in
the Maryland Primary Care Program (MDPCP) as a Care Transformation Organization
(CTO). A partnership between the state of Maryland and the Centers for
Medicare and Medicaid Innovation (CMMI), the Maryland Primary Care Program
is a statewide initiative to improve primary care services for Medicare
beneficiaries.
As a CTO, Frederick Integrated Healthcare Network, LLC provides administrative
support, analytics, program expertise, and care coordination services
to Maryland Medicare beneficiaries attributed to our partner participant
practices.
