of life for those with advanced and chronic medical conditions. Provide
symptom management for those with physical, emotional, and spiritual suffering
through patient centered relationships.
Who Benefits from Supportive & Geriatric Care? Anyone with chronic or life threatening illness experiencing symptoms due
to the illness itself or secondary symptoms as a result from treatment
for the disease.

Services
Symptom management – Management of pain, nausea, fatigue, difficulty breathing, changes
in appetite, depression, anxiety, or any other symptoms resulting from
disease or treatment for the disease
Communication and support for the patient and family – The team helps patients and their families make medical decisions
and choose treatments that are in line with their goals.
Social Work Support – Our social worker provides emotional support, referrals to community
resources and assistance with completion of advanced directives.
Pastoral Care Support – Pastoral counselors provide spiritual and emotional support.
