Outpatient Rehabilitation
Designed to offer restorative and medically necessary treatment in the comfort of your home.
When you need rehabilitative therapy and can’t get to an outpatient clinic, we bring care home to you to help restore functional ability and avoid future intervention or readmission.
What we offer
This program provides continued outpatient rehabilitation to help restore your full functional ability and independence. It includes:
- Speech therapy
- Physical therapy
- Occupational therapy
- Other services as needed
Who can qualify?
This program is for patients who still need skilled clinical care and no longer meet the criteria for intensive home care visits.

You do not need to be an existing or previous patient of Frederick Health Home Care.

Qualified patients will need an order for service from their provider.
Insurance & Billing
Insurance coverage for services in this program will vary from patient to patient. Check with your insurance carrier to see what is and isn’t covered.
