Primary Care

Our board certified Family Physicians, Internists, Nurse Practitioners and Physician Assistants at Frederick Health Medical Group Primary Care are focused on providing care when you are sick, managing a chronic condition or providing preventive care. Treatment through prevention is a major focus of your care. We are proactive at all ages to promote prompt annual examinations, screenings and age appropriate vaccinations.

Appointments

We offer daytime, evening and Saturday appointments. Call our access center to schedule an appointment at (240) 215-6310. Your access agent will ask specific questions to determine an appropriate appointment length. To request an appointment online, please fill out the form below:

Virtual/Telehealth Appointments

Frederick Health Medical Group Primary Care offers visits via your smart phone or laptop. Your provider will send you a link via text. This will direct you to a secure virtual visit. Examples of visits that may qualify for telehealth include fever, pink eye, sinus infections, rash, or lab/imaging review. Some chronic disease management may also be appropriate to schedule via telehealth. Inquire when scheduling your appointment.

Walk-In Appointment

We offer Walk-In Care. Walk-In locations are housed inside of our Urgent Care Locations. Upon arrival please tell the receptionist that you are a Frederick Health Medical Group Primary Care Patient since this a different service from Urgent Care. Walk-in locations do not manage chronic care, their focus is acute sick care for our established Primary Care patients. Appointments are not required for Walk-in care. (Toll House and Urbana Locations 8am to 8pm M-F and 8am to 6pm on weekends)

Online Patient Portal

Communicate directly with your care team through our online patient portal. With an email address, it is quick and easy to enroll. In addition the portal offers an option to request refills of your medications and review imaging and laboratory results.

Patient Centered Medical Home (PCMH)

Frederick Health Primary Care sites employ an innovative model of primary care called Patient Centered Medical Home (PCMH) where care is organized and evidence-based. We have been recognized by the National Committee of Quality Assurance (NCQA) as a Level 3 Patient Centered Medical Home (PCMH). We partner with you, your provider, and his/her care team work together to provide the highest level of patient care. Care Coordinators and care transition specialists enhance the quality and continuity of your care over time and across multiple health settings. Our philosophy is that by tracking your medical care and working with you individually to maintain your good health we may improve your compliance and overall health outcomes.
