The Comprehensive Care Center (CCC) offers a wider range of services. Many of these services are designed for individuals who have barriers to accessing care.

Anticoagulation Clinic
The CCC offers extensive warfarin (Coumadin) management. Our team of clinical pharmacists work closely with patients to provide education and optimize anticoagulation regimens.
Behavioral Health Care
Mental health is just as important as physical health. The CCC provides therapy and support groups as well as social work consultations when appropriate.
Cardiopulmonary Care
Caring for your heart and lungs is crucial to living a healthy lifestyle. The CCC offers a team-based focus on chronic diseases that impact cardiopulmonary health such as congestive heart failure, and COPD.
Diabetes Prevention & Managment
The CCC offers a wide range of options for individuals with diabetes. These include informational classes, consultations with nutritionists, and lifestyle coaching.
Geriatric Care
As we age, our bodies require different types of care. The CCC assists with advanced care planning and helps you establish your goals of care.
Nutrition Care
Many conditions are impacted by our diet and nutrition. The CCC offers nutrition therapy, educational information, and weight management resources.
Palliative Care
If you are facing a life-limiting illness, the CCC has a team-based approach to help you care for and manage your symptoms. These include home health visits and in person options.
Pharmacist Consultation
Our clinical pharmacists provide team-based, patient-centered services focusing on Medication Therapy Management and Collaborative Drug Therapy Management to patients with chronic diseases and also help reduce medication burden via deprescribing. They will collaborate with the medical provider to offer more in-depth education related to medication and optimize treatment plans to improve health-related outcomes. They also help the patient with medication affordability concerns by offering sample medication and helping patients with patient assistant applications.
Social Worker Consultation
Our social workers help patients, and their families navigate the medical system, assessing and monitoring patients' mental and emotional health. They assist patients with any concerns related to social and economic barriers and ensure our patients have adequate support at home. They help coordinate care, connect our patients to community resources, and facilitate access to healthcare by assisting patients in getting health insurance or applying for hospital/county financial assistance
Transitional Primary Care
Routine examinations, screenings, and visits with a Primary Care provider are the best ways to be proactive with your health. The CCC offers primary care services to those with no Primary Care provider, no insurance, and the under-insured.
