The Patient Portal is back online.

Please be aware that results and reports from January 27-February 18, 2025, may be found in "Documents". We appreciate your patience, as it may take time for all data to be fully uploaded. If you require immediate access to results or reports, please contact your ordering provider's office. For all patient-to-provider messaging, kindly allow up to three business days for a response. Thank you for your understanding.

The Frederick Health Patient Portal allows you to easily:

  • Review your lab & radiology results
  • View your upcoming appointments
  • View your medication and allergy lists
  • View your visit history
  • View discharge information
  • Message your provider
  • Telemedicine Visits
  • and more

Sign Up at any Frederick Health Facility

  • At the time of registration:
    • Provide a valid email address
    • Indicate that you want to access your health information online
  • A “Frederick Health Patient Portal - Password Reset” enrollment email from noreplyportal@frederick.health will be sent to the email address provided
  • Complete your enrollment using the instructions provided in that email.

Proxy for Patients

  • Complete the Proxy Access Request and Authorization form (Englishl Spanish)
  • Provide the completed form, along with the required support document(s) (see page 1 for your specific proxy request type), to the registration desk at any Frederick Health facility. You can also Fax to 240-566-3619 or mail to:

Apple Health and Other APIs

As a Frederick Health patient, you are able to connect all of your medical health records (from various institutions) by using Apple's Health app. This feature will gather all of your medical information and place it into one secure, easy-to-access location, alerting you when new information is available. All you need is an iPhone running iOS 11.3 or later.

Setup is simple:

  • Open the Health app on your iPhone.
  • Search for 'Frederick Health.'
  • Sign in with your Frederick Health Portal credentials.

If you would like to connect your medical information with other apps, please fill out our API request form here. Once we receive your request, we will work with the appropriate vendors to determine if they meet the technical requirements for a secure connection.
