We are pleased to provide our patients access to the Frederick Health Patient Portal. This convenient online service offers patients the flexibility to actively
engage in their healthcare via a secure, confidential web-based tool for
obtaining personal health information & direct sharing with providers.

The Frederick Health Patient Portal allows you to easily:

  • Review your lab & radiology results
  • View your upcoming appointments
  • View your medication and allergy lists
  • View your visit history
  • View discharge information
  • and more

Enrollment Types

Proxy for Patient:

  • Complete the Proxy Access Request and Authorization form (English l Spanish)
  • Provide the completed form, along with the required support document(s) (see page 1 for your specific proxy request type), to the registration desk at any Frederick Health facility.
  • An enrollment email will be sent to the provided email address; please allow up to 24 hours for delivery.
  • Complete your enrollment using the instructions provided in that email.

Enrollment as Self from Any Frederick Health Facility:

  • At the time of registration:
    • Provide a valid email address
    • Indicate that you want to access your health information online
  • A “Frederick Health Patient Portal - Password Reset” enrollment email from portal@frederick.health will be sent to the email address provided; please allow up to 24 hours for delivery.
  • Complete your enrollment using the instructions provided in that email.

From Home:

  • To enroll, you'll need your Frederick Health Medical Record number AND
    have a valid email address on file with Frederick Health.
    - If you do not know your Frederick Health Medical Record number, please contact the Frederick Health Patient Access department at (240) 566-3602.
    - If you’re unsure if you have an email address on file or need to update an existing email address, please contact the Frederick Health Patient Access department at (240) 566-3602.
  • If you have these qualifications, click Self-Enrollment to go directly to the enrollment web form.
  • Complete the enrollment web form and click Submit.
  • An enrollment email will be sent to the email address on file; please allow
    up to 24 hours for delivery.
  • Complete your enrollment using the instructions provided in that email.

*IMPORTANT NOTICE: Beginning December 1, 2020, the NextGen Patient Portal will no longer be available. To help maintain accurate health information and avoid any disruption of service, we ask that you please enroll in the Frederick Health Portal before the December deadline.

Patients of selected Frederick Health Medical Group specialities have access to the NextGen Patient Portal. You may use this portal to access information related to care provided by any participating speciality.

With NextGen Patient Portal You Can:

  • Review Patient Plans from your office visits
  • Review your Lab & Radiology Results
  • Send and Receive Secure Messages
  • View your Medication and Allergy Lists
  • View your upcoming Appointments
  • Schedule an appointment (where enabled by the speciality)

Enrollment is Easy:

  • During your office visit, provide your personal email address.
  • Indicate that you want to access your health information online.
  • An enrollment email will be sent to the provided email address.
  • Login to the NextGen portal using the temporary user id and password & link from that email.
  • Create your permanent user id, password & security question.
  • Explore your personal NextGen portal.
