Requirements for Successful Completion of Residency Program

In order to successfully complete the PGY-1 Frederick Health Pharmacy Residency Program, residents must meet the following requirements for pharmacy practice, deliverables, and achievement of objectives:

Pharmacy Practice

1. Residents shall complete their residency over a minimum of 52 weeks.
2. Residents shall complete 240 days or at least 2/3 of the residency (whichever is more) as a licensed pharmacist.
3. Residents shall complete a minimum of 72 staffing shifts which equates to every other weekend on both Saturday and Sunday starting the second week of residency plus one 4-hour evening shift every other week starting in August.

Deliverables

1. Residents must prepare or revise a drug class review, monograph, treatment guideline, treatment protocol, utilization management criteria, and/or order set as defined in Objective R1.4.2.
2. Residents shall successfully complete a major research project to meet Objectives R2.1.1-R2.1.6. Successful completion is indicated by
   1. Development and documentation of a project plan approved by RPD or designee as defined in Objective 2.1.2.
   2. Presentation of findings at a regional residency conference or comparable professional meeting.
   3. A final evaluation by the research project advisor.
   4. A written manuscript that meets guidelines for submission to a journal.
   5. A manuscript submitted to the program director by June 15^th.
3. Residents shall successfully complete a minor research project such as an MUE as defined in Objective R2.1.6.
4. To meet to meet Objectives 4.1.1-4.1.3
   1. Residents must present at the University of Maryland School of Pharmacy Pharmacotherapy Rounds or another CE program provided to health care practitioners
   2. Residents must create a written educational handout for patients, pharmacists, or other healthcare providers.
5. Residents shall complete a minimum of 6 hours of education requirement through the University of Maryland School of Pharmacy, nursing orientation, or other educational presentations given within or partnered with Frederick Health Hospital.

Achievements of Objectives

Residents shall obtain achieved for residency (ACHR) designation on at least 80% of the program’s objectives:

1. For objectives under goal R1.1 and 1.2 the resident must achieve (ACH) at least 3 times before obtaining ACHR designation.
2. For objectives under goal R 1.3, 1.4, 2.1, 3.1, 3.2, 4.1, and 4.2, the resident must ACH 2 times before obtaining ACHR unless it is attached to a deliverable which will supersede this requirement.
3. RAC grants ACHR designation after discussing the residents’ performance quarterly at the RAC meetings.
   1. For longitudinal learning experiences, only the final summative evaluation will be considered for ACHR designation.
   2. At any time during the course of the residency program training, if a preceptor and/or the RPD observe any resident performance as needing reinforcement, remediation, and/or further assessment, the RAC can decide to remove the ACHR rating from the associated objective(s) for further training and evaluation. If this occurs, it will be documented in the RAC meeting minutes, an action plan developed in collaboration with the resident which will be documented in the resident development plan and communicated with applicable preceptor(s).
4. The following scale will be used to define Needs Improvement (NI), Satisfactory Progress (SP), and Achieved (ACH):
   1. NI: The resident does not demonstrate competency or demonstrates minimal competency needing extensive intervention by the preceptor
   2. SP: The resident demonstrates competency and independence but needs moderate intervention and oversight of performance by the preceptor
   3. ACH: The resident demonstrates competency and completes activities independently with minimal preceptor oversight
